Free T3 Blood Test: What Your Results Mean
Free T3 (triiodothyronine) is the biologically active thyroid hormone that directly affects virtually every cell in the body. Most T3 is converted from T4 in the liver and peripheral tissues. "Free" T3 refers to the unbound fraction available to enter cells, as opposed to T3 attached to transport proteins. Free T3 is the most metabolically potent thyroid hormone and has a stronger, faster action than T4.

What Your Results Mean
A normal free T3 level indicates that the thyroid gland is producing adequate amounts of thyroid hormone and that conversion from T4 to T3 is occurring properly. Your metabolism, heart rate, body temperature, and energy levels are likely being regulated normally by thyroid function.
High free T3 indicates hyperthyroidism — the thyroid is overactive and producing too much hormone. Causes include Graves' disease, toxic multinodular goitre, thyroiditis, or excess thyroid hormone replacement. Symptoms include rapid heart rate, weight loss, heat intolerance, tremor, anxiety, and sweating.
Low free T3 occurs in hypothyroidism, where the thyroid is underactive. It can also appear in "non-thyroidal illness syndrome" (euthyroid sick syndrome), where serious illness reduces T4-to-T3 conversion as a protective mechanism. Symptoms of true hypothyroidism include fatigue, cold intolerance, weight gain, constipation, and depression.
